Of course, you should be capturing the most basic information from the client, including name, date, address, telephone numbers, what they want to work on, and whether they have tried hypnosis before. This might seem like a no brainer, but you would be surprised what hypnotists can forget about.
Their address is especially important given the fact that you can then send advertisements and mailing lists to them to inform them of any upcoming classes or products that you have in the future. Phone numbers can be just as important when it comes to emergencies or any changes to your schedule.
It is also important to have an understanding about what they want out of the hypnosis, as well as whether or not they have been hypnotized in the past. This can be one of the most important things that you have an understanding on, since it will let you know a number of things about the client. For one, if they have been hypnotized before and particular techniques work, you can know that starting there would be a wise choice for the both of you. The same goes for anything that was found to not be successful. It is important that you don’t make the same mistakes as other hypnotists that have treated them.
You should also be finding out information about any illnesses or medical conditions that they might have. This is partially because you want to avoid instant inductions with anyone that has a heart problem. Startling them could cause any number of problems that you would likely want to avoid. If someone is being treated for chronic pain, as well, it can be useful give them information that they can provide their normal physician with, just as a precautionary measure.

The general idea is that you are going to be gathering information to give you a basis to work off of. This foundation is important when treating people of all walks of life, since there are a lot of things that can be taking place in a person’s life, as well as where they are hoping for that life to go. Having a clear idea of goals is important, and you can’t create them without having an idea of where they currently are and where they have been.
Take the time to create an extensive intake form. It might be a minor annoyance for people the first time that they come to visit you, but it will lead to long term success in a way that no other first visit can touch. Talking to a person throughout their first session won’t have the same benefit as having them write everything down so that it can be quickly read over and assessed.
